Adderall (mixed amphetamine salts)

A prescription central nervous system stimulant with significant cardiovascular, psychiatric, and counterfeit-pill risks when misused.

01 · Classification

How Adderall works

Adderall is a mixture of dextroamphetamine and amphetamine salts. It is a central nervous system stimulant approved as a Schedule II prescription medication for ADHD and narcolepsy.

It reverses dopamine and norepinephrine transporters, causing substantial monoamine efflux into the synaptic cleft. It also inhibits monoamine oxidase and alters vesicular storage through VMAT2.

Excess norepinephrine produces alpha- and beta-adrenergic stimulation, accelerating heart rate and causing peripheral vasoconstriction.

02 · Acute effects

Effects by exposure level

Therapeutic / low

Heightened focus, wakefulness, reduced fatigue, and task persistence.

Elevated heart rate, mild blood-pressure increase, appetite suppression, and dry mouth.

High / non-prescribed

Hyperactivity, grandiosity, compulsive behavior, rapid speech, and irritability.

Tachycardia, palpitations, sweating, dilated pupils, and jaw clenching.

Acute toxicity

Severe paranoia, agitation, hyper-reflexia, tactile hallucinations, and acute delirium.

Malignant hyperthermia, arrhythmias, hypertensive crisis, and seizures.

Medical emergency

Recognizing acute toxicity

Severe agitation, paranoia, hallucinations, hyperthermia, chest pain, arrhythmia, hypertensive crisis, or seizures require immediate medical help.

Call 911 immediately for suspected stimulant toxicity. Do not leave the person alone or attempt to restrain someone who is delirious.

Clinical context

Medical use is not the same as misuse

Prescribed treatment is individualized and monitored. Taking more than prescribed, combining stimulants, or using an unverified product can change the risk profile dramatically.

03 · Long-term consequences

Health effects of prolonged heavy use

Neurochemical depletion

Prolonged supratherapeutic exposure can downregulate dopamine receptors, causing anhedonia, lethargy, and depression after cessation.

Stimulant psychosis

Prolonged sleep deprivation or heavy use can produce paranoia, hallucinations, disorganized thinking, and loss of contact with reality.

Cardiovascular strain

Excess norepinephrine drives alpha- and beta-adrenergic stimulation, increasing heart rate, vasoconstriction, and cardiac workload.

Counterfeit-pill hazard

Unverified pills sold as Adderall may contain methamphetamine, fentanyl, or nitazenes rather than amphetamine salts.
