01 · How it works

Classification and primary mechanism

Nitazenes are 2-benzylbenzimidazole synthetic opioids that act as ultra-potent, full agonists at mu-opioid receptors. Receptor activation inhibits adenylyl cyclase, closes voltage-gated calcium channels, and opens inward-rectifying potassium channels, profoundly suppressing respiratory neurons in the brainstem pre-Bötzinger complex. Metonitazene may rival fentanyl, while protonitazene and etonitazene can be several to many times more potent.

02 · Immediate effects

Recognize the effects

Signs of Use

  • Pinpoint pupils and heavy nodding
  • Slow, shallow, irregular, or absent breathing
  • Blue or gray lips, fingertips, or skin
  • Gurgling, choking, or inability to wake
  • Cold clammy skin, limpness, or sudden collapse

Short Term

  • Rapid opioid euphoria, analgesia, sedation, and emotional blunting
  • Progressive mental clouding, miosis, slowed heart rate, and loss of coordination
  • Severe respiratory depression from suppression of brainstem breathing circuits
  • Cyanosis, aspiration, coma, and cardiac arrest in severe toxicity
  • Tolerance and overdose risk can escalate rapidly with unknown potency

Long Term

  • Opioid use disorder and severe physical dependence
  • Withdrawal with pain, diarrhea, vomiting, sweating, insomnia, and agitation
  • Hypoxic brain injury after a nonfatal overdose
  • Infectious and medical harms from contaminated or injected products
  • Repeated exposure to unpredictable counterfeit pills and powders

03 · Long-term consequences

Extreme potency and counterfeit supply

Nitazenes were abandoned in pharmaceutical development because of excessive toxicity and now appear in clandestine drug markets. Uneven mixing creates lethal hot spots, and conventional fentanyl testing may miss them. A negative strip result does not make a powder or pill safe.

Medical emergency

Identifying an emergency

Call 911 immediately for unresponsiveness, slow or absent breathing, blue or gray skin, gurgling, collapse, or inability to wake. Give naloxone if available, provide rescue breathing or CPR when needed, place the person on their side if breathing but unconscious, and continue monitoring because repeated naloxone doses and extended observation may be necessary.

Treat suspected nitazene exposure like a severe opioid overdose: give naloxone, call 911, and start rescue breathing or CPR
